Transplant Coordinator - RN Beth Israel Lahey Health is seeking an experienced RN to coordinate transplant patient care.

Responsibilities

Coordinate information from referring physicians, testing and clinic visits for transplant recipients and living donors as defined in clinical pathways.

Provide educational information to recipients, donors and primary caregivers regarding all aspects of transplantation/donation including pre‑transplant, operative procedure, post‑operative management, long‑term follow‑up and research protocols.

Participate in medical and surgical outpatient clinics with collaborating physicians and surgeons; review outpatient lab values and testing results; work with physician/surgeon as needed to adjust care plan.

Coordinate and facilitate discharge planning with inpatient team to maintain continuity of care.

Provide accurate, appropriate and timely documentation to meet state and federal regulatory requirements.

Required Qualifications

Bachelor’s degree in Nursing.

Registered Nurse license.

American Heart Association – Basic Life Support Certificate.

3–5 years related work experience.

Basic familiarity with computers; ability to navigate web‑based applications at a basic level.

Preferred Qualifications

Experience in a teaching hospital and specialty area expertise.

Ambulatory or transplant experience.

Acts as team leader for small projects or work groups, creating collaborative and respectful team environment.

Customer Service:

Provides high level of customer service and staff training to meet customer service standards.

Physical Nature of the Job Sedentary work; exerting up to 10 pounds of force occasionally in carrying, lifting, pushing, pulling objects. Sitting most of the time, with walking and standing required only occasionally.

Compensation Hourly wage range: $39.14 – $101.14.
