Nurse Practitioner - PRN/Casual - Flexible Schedule - Join our fun & supportive team! Ecumen Hospice is hiring a Nurse Practitioner for its location in Duluth, Minnesota. Ecumen Hospice is a non‑profit, Medicare‑certified hospice that has been providing life‑honoring care in Minnesota for almost 30 years. We serve those who are experiencing a life‑limiting condition and the people that are closest to them, through a comprehensive offering of services which include but are not limited to: physician oversight, nursing, social work, spiritual care, hospice aide assistance, music therapy, massage therapy, volunteer services, pet therapy, and bereavement care.

The Hospice Nurse Practitioner travels to the hospice patient and performs hospice face‑to‑face visits prior to the third and later hospice certification periods by assessing hospice eligibility. The Hospice Nurse Practitioner will provide clinical information to the hospice medical director for interpretation for eligibility and make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.

Schedule: On Call / Casual / PRN / Pick up shifts as they become available.

Pay: The targeted pay range for this job is $50.00 to $60.00 per hour.

Essential Job Responsibilities

Complies with all state and federal laws and regulations related to providing hospice care.

Complies with Ecumen hospice program’s policies and procedures, and help Ecumen continually evaluate and improve, when prudent, its policies and procedures. Participates in the hospice program’s quality assessment and performance improvement program.

Manages symptoms of terminal illnesses by developing an individualized care plan, prescribing medications/treatments, and obtaining consultations and appropriate referrals.

Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.

Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.

Assumes the medical component of the hospice patient’s care program, and assumes responsibility for the palliation and management of the terminal illness and conditions related to the terminal illness.

Assesses hospice patients by reviewing the clinical information about the patient and when appropriate, provides documentation that it is anticipated that the patient’s life expectancy is 6 months or less if the illness runs its normal course.

Prepares, in consultation with the patient’s attending physician, if any, and the hospice IDG, and patient or representative, an individualized written plan of care and services for each patient that reflects patient and family goals and interventions based on the needs identified.

Nurse Practitioner Minimum Required Qualifications

Graduate from an accredited School of Nursing and accredited Nurse Practitioner Program

1-3 years related experience

Current nursing licensure in State of Minnesota and CPR certification

Accreditation from an approved certifying body for advanced practice nursing

Ability to communicate effectively in both written and verbal formats

Valid driver's license and proof of vehicle insurance

Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Prior experience working in Hospice
