Job Overview:
We are seeking a skilled Physical Therapist to join our team. The ideal candidate will have a passion for providing quality care and rehabilitation services to patients in need. Different from other Home Health Agencies, we provide documentation assistance with AI cutting time to complete Oasis Documents in half, allowing you more face‑to‑face time with the patient and more thorough documentation.
Responsibilities:
Conduct patient assessments and develop individualized treatment plans
Provide hands‑on therapy and exercises to promote physical recovery
Educate patients and their families on at‑home exercises and self‑care techniques
Collaborate with other healthcare professionals to ensure comprehensive patient care
Maintain accurate patient records and documentation
Stay current on advancements in physical therapy techniques and technologies
Qualifications:
Degree in Physical Therapy from an accredited program
State licensure as a Physical Therapist
Experience in hospital medicine, geriatrics, acute care, and orthopedics preferred
Strong knowledge of anatomy and physiology
Excellent patient care and communication skills
