About Us Safe Life Home Health Care, LLC was founded in 2010. We are a professional, customer‑centric home health agency recognized by state licensure, CHAP accreditation, and Medicare certification. Our multidisciplinary team provides comprehensive care across 13 Illinois counties.
Position Overview Provides Occupational Therapy services to patients as directed by Plan of Care, oversees OT Assistants, and manages follow‑through of OT procedures.
Responsibilities
Assists the physician in evaluating patients’ level of function, establishing treatment plans and goals.
Performs initial evaluations within 48 hours of referral and re‑evaluations every 30 days, coordinating plans of care with the physician as needed.
Prepares clinical and progress notes and required summaries.
Revises treatment plans and contacts physician for changes in plan of care.
Advises and consults with patient’s family and other health care staff.
Coordinates care with agency DON and other staff as appropriate.
Observes, records, and reports any changes in the patient’s condition and updates the agency on status or schedule changes.
Supervises the Occupational Therapy Assistant.
Instructs patient and/or family in use of equipment, safe patient management, and home therapy programs as needed.
Communicates effectively with patients, caregivers, families, and agency staff.
Provides quality care and reports issues or concerns to the DON/Administrator.
Assures that all tasks are performed in a timely manner.
Checks in on Occupational Therapy Assistants every 4–6 patient visits.
Qualifications
Licensed Occupational Therapist by the State of Illinois.
Minimum one‑year Home Health Care clinical experience.
Valid driver’s license with proof of insurance coverage.
Maintains annual TB test, CPR certification and other required certifications.
Able to read and interpret instructions related to patient care.
Proficient at prioritizing multiple tasks as needed.
Must be able to travel to patients’ residences.
