Description

Maine Medical Center. Professional – Clinical. Req #: 60308. For a limited time MaineHealth is offering up to $5,000 sign‑on bonus for all eligible Social Workers. Current MaineHealth member employees are ineligible; former MaineHealth members ineligible until more than 3 months separation. Bonuses may be prorated based on schedule.

~ 40 Hours/week, full‑time, day shift.

Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW) is required.

Summary

The Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team delivering patient‑ and family‑centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize quality of life and align the individual's medical care with what matters most to them.

Key responsibilities:

Evaluate psychosocial needs of patients and families.

Provide targeted counseling, including anticipatory grief support.

Facilitate family meetings and goal‑setting discussions.

Make referrals to community resources.

Engage in effective interdisciplinary team collaboration.

Work primarily in the outpatient clinic in Scarborough; may meet patients at hospital or other specialty settings through outreach (in person and/or telehealth).

Additional responsibilities include:

Diagnose and treat patients/clients with individual, group, and family therapy.

Develop complex discharge planning and coordinate aftercare services.

Make referrals for community services and consult treatment team regarding patient/family social and environmental needs.

Supervise other social workers, students, or staff as appropriate.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

Education:

Master’s Degree in Social Work/Clinical Counseling from an accredited institution.

License/Certifications:

Current state license as LCSW or LMSW‑CC.

Experience:

Two years of relevant health or mental health experience preferred; demonstrates competency in all essential functions of a Social Worker I role.
