The Admission Coordinator develops and maintains relationships with referral sources and provides patient assessments in various settings to facilitate a smooth transition into Hospice of the Valley programs. Care delivery primarily occurs in hospital settings and skilled nursing facilities. Responsibilities

Facilitates smooth transition and admission of patients from health care settings into HOV programs. Promotes HOV services and programs. Develops and maintains therapeutic relationships. Plans care and intervenes effectively. Provides effective patient/family/caregiver teaching. Supports the death/dying process. Collaborates in providing patient care. Creates timely and accurate documentation. Utilizes resources effectively and efficiently. Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services. Maintains and enhances professional skills. Adheres to high standards of personal and professional conduct. Minimum Qualifications

Graduate of accredited school of nursing. Minimum one year of recent experience in medical, surgical or critical care as a professional nurse. Strong clinical assessment skills. Excellent communication and interpersonal skills. Solution driven, creative and resourceful problem solving skills. Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. Preferred Qualifications
