The Phoenix Foundation is seeking an
experienced Case Manager
with a strong background in
homeless services, housing navigation, and supportive housing programs
. This position is a
quick-hire opportunity
for professionals who can step in confidently and manage comprehensive casework with
a manageable caseload of approximately 15 participants , including high-barrier populations.
This role focuses on
assessments, individualized service planning, housing placement, HMIS documentation, and ongoing advocacy
to support individuals and families experiencing homelessness as they work toward stability and permanent housing.
Salary:
$45,000 to $50,000 annually
