LPN Resident Care Coordinator Position
This is a supervisory position in assisted living. Experience required. The position is full time, 2p-10p shift, with weekend availability. Up to $30.00/hour plus shift differentials based on experience.
Responsibilities:
Assist with medications when needed.
Assess residents and work with physicians to ensure best healthcare for residents.
Supervise staff and ensure excellent customer service.
Assist with admissions and discharges.
Respond to residents’ needs in a timely manner with memory care residents and families.
Conduct assessment, care planning, audits, family council involvement, and related tasks.
