Full-Time RN Clinical Manager – Hospital at Home Program
AccordCare is seeking an experienced RN to lead our Hospital at Home Program. This role combines field-based patient care with clinical leadership responsibilities.
Location Stuart to Vero Beach, FLHybrid)
Schedule Monday–Friday, 8:00 AM – 5:00 PM
Salary $72,000 annually + comprehensive benefits

Support and mentor field clinicians to ensure quality care delivery
Maintain compliance with all regulatory and accreditation requirements
What You’ll Bring
Active RN license in Florida (required)
Minimum 1 year of home care experience (preferred)
Experience with Hospital at Home programs (highly preferred)
Valid driver’s license and reliable transportation
Strong communication, leadership, and time management skills
EMR proficiency; AlayaCare experience a plus
