Full Time Occupational Therapist / OT Home Health Responsibilities
Evaluate and address the needs of individuals in their homes to enable safe and productive living environments. Home safety medication management, wheelchair seating and positioning, pain management, falls prevention, and chronic disease management. Dementia family caregiver training and development of individualized care plans. Educator, patient advocate, and member of the interdisciplinary care team. Qualifications
Current unrestricted CA Occupational Therapist license. Current BLS/CPR certification. Valid CA Driver'll License. Knowledge of local community resources. Knowledge of individual development and human behavior as it relates to health care. Knowledge of state and federal homecare regulatory guidelines preferred. Experience in home health care highly preferred; high acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case Management fundamentals highly desired. Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized. Benefits
