Infusion Nurse II Option Care Home Health LLC

Overview Option Care Health, Inc. is the largest independent home and alternate site infusion services provider in the United States. As an Infusion Nurse II, you will coordinate and provide intermediate direct patient care in the home or infusion suite, ensuring patient safety and compliance under physician care.

Job Responsibilities

Initiate, develop, and implement intermediate nursing plan of care treatments; evaluate patient progress toward goals. Organize and participate in direct patient care, administer medications, and educate patients and families.

Assess patient needs and physical status at each visit; reevaluate needs through reassessment, response to therapy, and supplemental orders.

Obtain and clarify physician orders for treatment revisions, informing physicians of significant changes promptly and providing written summaries.

Provide training and mentorship to Infusion Nurse I’s as needed to ensure safety and compliance.

Teach patients and families using a patient‑centered approach to achieve independence with prescribed therapy.

Communicate with Option Care’s clinical team, providers, patients, and families to facilitate continuity of care. Prepare real‑time documentation during visits and submit to the nursing department within billing timelines.

Coordinate discharge planning and prepare discharge summaries with patient instructions and reports when transferring care.

Demonstrate compliance with agency operations, policies, professional standards, local, state, and federal regulations, and accreditation standards.

Maximize work efficiency using computers and automation technologies to validate orders and communicate patient care.

Respond promptly to patient requests and initiate emergency procedures when necessary.

Accept accountability for own practice through ethical conduct and adherence to policies reflecting Option Care’s mission and values.

Observe legal and ethical guidelines for safeguarding patient confidentiality, including HIPAA compliance.

Explain Option Care’s scope of services and financial obligations to patients.

Attend required training modules, in‑services, and continuing education to maintain competency and licensure. Provide oversight and coordination of paraprofessionals; may act as supervisor when necessary.

Participate in multidisciplinary team conferences and provide training and mentorship to other nurses for orientation and supervisory activities.

Participate in nursing department on‑call responsibilities.

Perform other related duties as directed by supervisor.

Basic Education and Experience

Active and unrestricted RN license in the state of practice.

Minimum of 1 year of nursing experience.

Current CPR certification required.

Basic Qualifications

Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs.

Required licensure to operate a motor vehicle in the state of practice with access to a vehicle and proof of liability insurance.

Basic knowledge of computer operating systems and software applications; ability to apply technology tools to communicate and document care.

Preferred Qualifications

Bachelor of Science in Nursing from an accredited school.

1–2 years of previous infusion nursing experience.

Physical Demand Requirements

Ability to lift up to 50 pounds (maximum 75 pounds); exert up to 20 pounds of force occasionally or 10 pounds frequently. Physical demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.

Likelihood of occupational exposure to blood, body fluids, and other potential infectious materials; exposure to hazardous substances and unpredictable home environments.

Safety requirements: closed‑toe shoes, face mask, goggles, gown and/or gloves, and functional communication devices for routine and emergency situations.

Travel Requirements

Willing to travel up to 100% of the time for business purposes.

Salary and Benefits Pay Range: $38.84–$64.72 per hour (determined by education, experience, knowledge, skills, and abilities).
