Summary The Licensed Practical Nurse (LPN) provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM within the specified geographical territory. The LPN educates patients and their family members and ensures their safety. The LPN is accountable for accurate documentation and remains compliant with all legal rules and regulations. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.

Qualifications & Attributes
Licensed Practical Nurse with current license in the state of employment
Minimum one year of experience as an LPN in an acute care setting (two years preferred). Home‑care experience desired.
Effective communication and the ability to create positive impressions with patients, families, physicians, and co‑workers.
Ability to remain calm, patient, accommodating, compassionate and caring.
Knowledgeable on nursing best practices.
Ability to make appropriate nursing judgments and identify situations and handle them with the best possible solutions.
Detail‑oriented and observant.
Disciplined work ethic with the ability to prioritize and be timely.
Ability to follow directions and work as a team member.
Valid driver’s license and auto insurance with your name as a listed driver.
Major Areas Of Responsibility
Provides skilled nursing care in a patient’s home as ordered by the attending physician.
Performs care by following order sets and goals made by RN.
Observes and monitors patient conditions.
Notifies RNCM of any changes in patient condition.
Administers medication as prescribed by the physician.
Counsels patient and family on disease/injury processes and management.
Incorporates patient and family in development of plan of care.
Helps decrease re‑hospitalizations by teaching patients preventative measures and good decision‑making.
Listens to patient and family members.
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
Documents all aspects of treatment, assessments, and patient education.
Maintains active LPN license in state of employment.
Communicates with scheduler regarding any changes outside normal.
Participates in all on‑call requirements and case conferences.
Attends in‑service trainings and mandatory agency meetings.
Maintains a professional appearance and adheres to agency policies and procedures.
Essential Functions
Drives to patient’s primary location per scheduled visit.
Daily attendance at assigned visit locations.
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines; completes and submits all required documentation within specified company requirements.
Knowledgeable with providing skilled care within the scope of practice for an LPN.
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
Compensation per visit reflects a lump‑sum payment for all visit activities including preparation time, travel, lab drops, charting, and communication.
Health Qualifications
Must be able to meet all physical position requirements; evidence of current negative tuberculin skin test or x‑ray or TB testing at orientation (annual thereafter).
Employees must be free of physical/medical conditions that limit or restrict ability to perform job functions; may be at risk for exposure to bloodborne pathogens.
