Overview Texas Health and Human Services Commission (HHSC) seeks a Qualified Intellectual Disability Professional III to coordinate the Individual Support Plan (ISP) process for individuals with intellectual and developmental disabilities at a state-supported living center. The role provides interdisciplinary leadership, ensures compliance with ICF/IID standards, and promotes client well‑being.

Responsibilities

Orchestrate and oversee the ISP process for the assigned caseload, including facilitation and documentation of IDT meetings (ISP preparation, addendum, annual ISP).

Ensure all individuals receive assessment and evaluation per IDT determinations for current functioning, preferences, strengths, and needs.

Coordinate and facilitate participation of the individual and LAR with paraprofessional and professional staff in development and delivery of needed supports.

Follow up on recommendations for supports, services, equipment, or programs to meet individuals’ needs in a timely manner.

Observe and interact with individuals across settings and program areas.

Collect and analyze data on ISP implementation and effectiveness; document information and coordinate with IDT for modifications as needed.

Train, coach, and mentor staff on behavior management and implementation of supports; provide leadership and ad hoc training to all levels of staff.

Work with behavioral health department staff to ensure behavioral needs are met and supports are integrated into the ISP.

Monitor and evaluate monthly implementation and documentation of ISP plans, programs, supports, and services; provide analysis in the QIDP monthly review document.

Ensure proper client protections and respond to human rights committee process for restrictive practices.

Maintain electronic records documenting cross‑disciplinary services and complete required documentation in compliance with ICF/IID standards.

Participate in agency disaster response or continuity of operations activation as assigned.

Qualifications

Bachelor’s degree in psychology, sociology, social work, special education, rehabilitation counseling, or related human services field; equivalent coursework may be reviewed.

At least one year of experience working directly with persons with intellectual or developmental disabilities.

Knowledge of IDT process and people‑centered planning; knowledge of rehabilitation and skills acquisition programs.

Ability to coordinate programmatic and clinical services and supports; ability to prepare and maintain records.

Strong communication skills, both verbal and written; ability to assess and solve problems.

Organizational skills and ability to prioritize and adapt to change; ability to work independently and harmoniously with staff and LARs.

Physical ability to perform direct care work as needed, including emergency procedures.

Computer proficiency: personal computer, electronic records systems, and databases.
