The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus‑based direct services and supports at 13 centers located throughout the state.

Senior Direct Care Registered Nurse with Associate’s Degree in Nursing (ADN) performs highly complex (senior level) nursing work providing assessment, care and integrated treatment of individuals residing at the SSLC. Performs nursing tasks, procedures, and assessments, administers medications, immunizations and treatments in accordance with HHSC, SSLC and state regulations. Interprets diagnostic tests, collects specimens, communicates findings and recommendations, coordinates nursing services, monitors health of individuals, provides consultation, training and technical assistance, develops in‑service training, and leads and mentors other staff.

Essential Job Functions

Attend work on a regular basis; may be required to work rotating schedules, extended shifts and overtime.

Perform highly complex and technical nursing tasks – e.g., tracheostomy, gastrostomy/jejunostomy, wound care, breathing treatments.

Administer medications, immunizations and treatments; observe patients for unusual symptoms and reactions.

For new admissions, provide detailed initial assessment in collaboration with RN Case Manager.

Interpret diagnostic tests, identify and assess patients’ responses, collect specimens and conduct screening tests.

Coordinate and organize nursing services and activities; monitor weight, diet, feedings, vital signs, and signs of illness; initiate acute care plans as needed.

Provide consultation, training and assistance to direct support professionals (DSPs).

Assume leadership and oversight within assigned units; act as preceptor for other registered nurses; train new nurses; serve as acting Nurse Manager when required.

Support Nurse Managers with supervising drug counts, medication logs, emergency checklists, temperature logs and glucose controls.

Assist with chronic care plans, injury reports, restraints and medication variances; monitor completion of interventions.

Document and report treatments and observations in the electronic medical record (EMR) accurately and timely.

Work independently with all levels of staff; collaborate with infection control, quality assurance and therapeutic nursing programs.

Participate in facility quality assurance/improvement initiatives; provide recommendations for process improvements.

Analyze data and assess nursing outcomes; provide input to the interdisciplinary team (IDT).

Follow local SSLC policies and protocols; communicate diagnoses and treatment plans to staff and guardians/LARs.

Respond to emergency situations per protocols; provide educational and safety training to staff.

Serve in disaster response, continuity of operations (COOP) activation and other agency‑wide duties.

Licensure and Certifications Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Initial Screening Criteria

Associate degree in nursing (ADN) is required.

At least two years of experience as a practicing registered nurse in a health agency, hospital, or health‑care facility.

Experience in assessment and documentation of health status is required.

Specialized training or experience working with individuals with complex medical and behavioral health issues is preferred.

Experience working with individuals with Intellectual and/or Developmental Disabilities (IDD) is preferred.

Additional Information Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE). Flexibility in work hours may be required; the position may require overtime, extended hours, weekends and holidays.
