Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services throughout Arizona.
Discharge Supervisor Discharge Supervisor is responsible for overseeing all aspects of discharge for patients receiving services in Crisis, Inpatient, or Transition Point programs. The role actively involves patient discharge planning from the time of admission and ensures that family members, enrolled providers, and other identified supports are engaged. The supervisor attends 4‑hour staffing sessions and daily huddles with the multidisciplinary team to identify potential discharge barriers and provide feedback on the current discharge plan. The supervisor ensures the discharge transition plan accurately reflects the discharge plan including placement name, address, outgoing referral and follow‑up appointments. The supervisor works directly with the Clinical Manager, Clinical Director, and Operations team to ensure that discharge planning and services are effective and efficient in addressing patient and community needs in accordance with CBI Clinical Policy and Procedure, Arizona Administrative Code, ADHS/DBHS Provider Manual, Code of Federal Regulations, and contract requirements.
Education and Experience
Bachelor’s degree in related field required, Master’s degree preferred.
1–3 years of full‑time behavioral health care related work experience preferred but not required.
Fingerprint clearance card required.
Current Arizona driving licence.
Clear 39‑month motor‑vehicle record.
