Responsibilities
Identify patients with barriers to a safe discharge plan.
Complete psychosocial assessments on all patients.
Document all clinical assessments, notes, and referrals in Epic.
Complete screens and coordinate all new skilled nursing facility and rehab facility referrals.
Assess cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.
Educate patient/family about post-acute care options and the role of choice.
Educate patient/family about managed care and payer issues.
Preferred Criteria
At least one year of experience in an acute care setting.
Discharge planning experience.
Experience with Epic and Allscripts.
Qualifications
Master’s Degree in Social Work (MSW).
New York State License as a Licensed Masters Social Worker (LMSW).
Communication and negotiation skills.
Ability to work independently and collaboratively with the interdisciplinary team.
Work efficiently and effectively under pressure.
Computer literacy.
Salary $44.16-$53.81 per hour.
