Responsibilities As a Home Health PT at Inspired Homecare, you will conduct comprehensive assessments, create personalized treatment plans, deliver high‑quality care in patients’ homes, monitor patient progress, adjust treatment plans as needed, and provide ongoing support to help patients achieve their goals. You will work closely with a diverse healthcare team to ensure seamless coordination of care.
What matters most
A degree in Physical Therapy from an accredited university/program.
A current, unrestricted state license as a Physical Therapist in the state of practice.
Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
Strong organizational and communication skills.
Experience in Home Health Therapy and completion of OASIS documentation is preferred.
