Job Summary:
UofL Health is an integrated regional academic health system serving over 12,000 team members. The Licensed Practical Nurse (LPN) will provide patient care utilizing the nursing process, collaborating with interdisciplinary team members to promote patient recovery and functional improvement in a post‑surgical/step‑down setting.
Responsibilities
Demonstrate knowledge of nursing theory and practice within the scope defined for LPNs by the Kentucky Board of Nursing.
Perform a comprehensive daily assessment of assigned patients and document findings in patient records.
Prioritize care based on assessment and monitor progress toward goals.
Review and revise patient care plans per established policies.
Deliver direct patient care per physicians’ orders and the care plan, documenting interventions.
Monitor laboratory values and report abnormalities within 30 minutes.
Administer prescribed medications according to schedule and procedure.
Implement physician and nursing orders with initiative without direct supervisor instructions.
Assist in patient education, ensuring families are instructed on self‑care and follow‑up.
Communicate effectively with interdisciplinary team, reporting status and changes per policy.
Maintain a supportive, positive environment for patients and families.
Escalate unresolved issues to appropriate personnel promptly.
Demonstrate age‑appropriate care, including pediatric and geriatric pharmacology.
Calculate dosage adjustments for geriatric patients and adjust for renal function.
Participate in special projects, maintain clean patient care areas, and ensure equipment functionality.
Follow universal precautions and safety procedures, contributing to performance‑improvement activities.
Attend shift meetings, review minutes, and provide nursing literature insights.
Maintain compliance with health and safety requirements, complete annual competencies, and stay current with company policies and HIPAA privacy.
Perform additional duties as assigned.
Shift Requirements Shift length: 8–12 hours per shift; 3–4 shifts per week (36–40 hours for full‑time).
Qualifications
Completion of an accredited LPN program (required).
Current license to practice as an LPN in Kentucky (required).
CPR/BLS certification upon hire (required).
Prior acute‑care or rehabilitation experience preferred.
Additional Information This role is full‑time and entry level.
