At Lakeside Health & Wellness, we are launching an In-House Rehabilitation Program providing advanced treatment technologies and interventions by highly skilled Physical, Occupational & Speech Therapists. We seek a PRN Physical Therapist to deliver excellent post-acute care and collaborative rehabilitation services.
Physical Therapist (PT) – PRN
Responsibilities
Facilitate rehabilitation of patients with emotional, physical or mental disabilities.
Implement occupational therapy care plans.
Coordinate care with the entire care team.
Educate patients and family members.
Prepare timely, complete and accurate clinical notes.
Communicate effectively with patients, caregivers, healthcare providers, office support and other care team members.
Qualifications
Believes in a team approach to problem solving and care.
Licensed Physical Therapist in the State of Texas.
Experience in Skilled Nursing, Long Term Care or Veterans Home is highly preferred.
Knowledge of Home Health regulations and general healthcare compliance.
Experience with EMR.
Truly cares for people and will go above and beyond to provide outstanding patient care.
Passion for therapy and teaching.
