Responsibilities

Perform comprehensive patient assessments and OASIS documentation

Initiate individualized care plans

Recommend and coordinate additional services

Communicate with providers and caregivers to ensure safe transitions

Qualifications

Minnesota RN license in good standing

Admission and OASIS experience

Strong communication and time management

Reliable transportation

Army: 66H, 66N
