Location: East Bethel, MN Campus

Job Type: Full‑time (M-F 2:00pm - 10:00pm, occasional weekend and holiday support).

Position Summary Works collaboratively with the care team to develop and implement a detailed care plan supporting youth transitioning from residential treatment to discharge and placement, championing youth advocacy and empowerment.

Primary Responsibilities

Meet with youth one on one to build trust and rapport, listening intently to their history and discharge readiness.

Discern, learn, and understand youths’ experiences to provide relevant information to the care team and integration into care plans.

Collaborate with the care team to source placement opportunities suited to the youth, interfacing with families, foster caregivers, or alternate residences.

Use influence to encourage caregivers and treatment facilities to consider the youth’s progress and needs; educate them on program resources.

Collaborate with the care team to develop and implement a comprehensive care plan, connecting youth to appropriate resources.

Maintain ongoing consistent communication with care team members, program management, and youth.

Support youth in developing and enhancing skills—resume writing, job searching, interview preparation, organizing finances, prioritizing schoolwork, obtaining a driver’s license, etc.

Provide individualized support to break barriers to employment, education, relational, leadership, citizenship, social, and financial goals.

Integrate the goals and objectives of the care plan into daily touchpoints, conversations, and tasks, linking youth to community resources.

Actively listen to youth, empathize, relate, and focus on life‑skills enhancement and coping mechanisms.

Develop trust and rapport, drawing upon personal lived experiences while maintaining professional boundaries.

Foster an encouraging relationship, balancing clear boundaries and challenging clients for growth, independence, and self‑confidence.

Serve as a champion and advocate for each youth, highlighting progress and providing encouragement and empowerment.

Leverage the youth’s care plan by conducting check‑ins via phone and/or in-person visits; observe successes and challenges and relay information to the care team.

Qualifications & Requirements

High School Diploma or GED.

Demonstrated lived experience coping with adolescent challenges (e.g., foster care, residential treatment, juvenile justice).

Demonstrated interpersonal maturity and emotional intelligence; ability to build trust and set healthy boundaries.

Ability to build strong relationships with diverse individuals or communities and understand their unique needs.

Willingness to meet the cultural needs of individuals or communities, considering language, traditions, and values.

Valid driver’s license and access to reliable transportation (must meet state regulatory agency requirements).

Preferred Education and Experience

Bachelor’s Degree.

Previous experience working with youth who have experienced trauma and/or a mental health crisis.

Physical Demands Employee must regularly talk and hear, have close vision; may require extended periods of sitting, bending, moving, lifting, and carrying materials; must be able to lift and carry 50+ lbs. Reasonable accommodations may be provided.

Schedule / Pay / Location

Full‑time, on‑site opportunity – M-F 2:00pm – 10:00pm (occasional weekend and holiday support).

Pay rate: $21.00 – $23.00 per hour.

Location: 900 189th Ave NE, East Bethel, MN 55011.

Salary $21 – $23 per hour.
