Registered Nurse (RN) Home Health – Crosby, Highlands
Join Transition Home Healthcare, an elite home health agency serving Houston, TX and surrounding areas since 2010. We are looking for a dedicated Registered Nurse (RN) to provide high‑quality care in the home setting.
Shift Available:
PRN
Service Area for Coverage:
Crosby, Highlands
Responsibilities
Oversee nursing care team.
Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
Plan, organize, and direct home care services.
Conduct assessments of patient care needs and develop care plans.
Educate patients and their families on managing their health conditions, medication regimens, and necessary lifestyle changes.
Prepare timely, complete, and accurate clinical notes.
Qualifications
Valid RN license in the State of Texas.
At least 1 year of home health experience.
Knowledge of home health regulations and general healthcare compliance.
Experience with Home Care Home Base (HCHB) EMR preferred.
Excellent communication and people skills.
Ability to work independently and as part of a team.
