Job Summary The pharmacist will provide a variety of pharmacy services for the inpatient hospital and outpatient infusion center. While servicing these patients, the pharmacist is responsible for ensuring safe and effective use of medications within the hospital. Direct collaboration with other multidisciplinary groups within the hospital is needed to optimize patient outcomes. All pharmacist duties will be under the general direction of the Pharmacy Director or other Pharmacy Leadership.
Key Responsibilities
Review and verify medication orders for accuracy and appropriateness, including contraindications, drug‑drug interactions, drug‑food interactions, allergies, appropriate dose, and therapeutic duplications.
Prepare, dispense, and distribute medications to inpatient and outpatient areas.
Monitor patient medication through a Pharmacy Dosing Service and according to institution policies and procedures.
Review and interpret laboratory results and information in patient charts accurately.
Provide accurate and timely drug information to multidisciplinary teams within the hospital.
Oversee sterile compounding and IV admixture preparation.
Support antimicrobial stewardship, formulary management, and therapeutic alternative initiatives.
Document clinical interventions and clinical dosing adjustments accurately.

Requirements
Education: Requires knowledge of theories, principles, and concepts normally acquired through the completion of a PharmD degree (preferred), B.S. in pharmacy, or successful completion of the foreign pharmacy equivalency exam. Residency is preferred but not required.
Licensure: Must hold a valid Indiana license or be a graduate of an accredited school of pharmacy and licensed in the State of Indiana as a Pharmacy Intern.
Experience: Associate must have proficiency in the practice of pharmacy with an emphasis on pharmacokinetic monitoring, nutritional consults, and drug information services that is usually obtained from graduation of an accredited college of pharmacy or passing a pharmacy foreign equivalency examination and two years of hospital/institutional pharmacy practice experience or completion of a pharmacy practice residency.
