JOB DESCRIPTION

In alignment with Gallaher Signature Living (GSL) mission and values, the Health & Wellness Director is responsible for overseeing and coordinating resident care services under the medical direction and supervision of residents’ attending physicians to ensure residents maintain an active, independent, and dignified lifestyle. This position partners closely with residents and their families to support the physical and emotional well-being of residents and serves as a liaison with hospital personnel, physicians, community organizations, and other healthcare-related service agencies.

The Health & Wellness Director monitors, evaluates, and assists in modifying treatment regimens to meet the physical and psychosocial needs of residents, in accordance with established medical practices, state regulations, and the policies and high-quality standards of Gallaher Signature Living.

Compensation:

$110,000-$130,000/ year

Key Responsibilities

Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies to coordinate and advocate for resident care needs.

Perform nursing duties within the scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by physicians.

Ensure accurate, timely, and thorough documentation of resident care and participate in end-of-shift reporting to ensure continuity of care.

Oversee and promote proper use of the Communication Log on each shift.

Coordinate incidental medical and dental care for residents.

Assist the Executive Director or Journey Program Director with physician orders and pharmacy coordination, including organization of central medication storage and delivery for each shift.

Observe residents and proactively identify changes in condition, communicating relevant information to physicians and healthcare professionals as needed.

Ensure proper documentation of changes in condition, incidents, and required reports, maintaining compliance with regulatory standards and medical record requirements.

Provide guidance and support to Care Managers and Medication Aides and assume direct care or medication aide duties when necessary.

Respond promptly to residents and staff needs and carry a pager and radio at all times while on property.

Assist residents with activities of daily living (ADLs), including grooming, dressing, oral hygiene, toileting, bathing, incontinence care, feeding, nail care, transferring, and mealtime assistance, as needed to support quality of care.

Follow Universal Precautions and adhere to all infection control and exposure policies.

Promote resident dignity, safety, privacy, and confidentiality at all times.

Display tact, professionalism, and compassion when interacting with residents, families, visitors, and coworkers.

Assist new residents and families with orientation, supporting a smooth transition into the community.

Encourage resident participation in daily routines and activities that promote independence.

Collaborate with the Lifestyle Department to support planned programs and activities.

Attend all required in-service trainings and continuing education programs.

Assist the Executive Director or Memory Care Director with staff orientation and training, as requested.

Support community marketing efforts through professional engagement during tours, inquiries, and public interactions.

Maintain a safe and secure environment by conducting daily and nightly rounds, reporting safety concerns and maintenance needs promptly.

Foster teamwork through cooperative and respectful interactions across all departments.

Uphold a positive, professional image through appearance, conduct, and communication.

Requirements

Current Registered Nurse (RN) license in good standing

Prior experience in a healthcare environment, preferably in senior living or with cognitively impaired older adults

Ability to perform mathematical calculations related to medication administration

Ability to work patiently and compassionately with cognitively impaired individuals

Ability to assess stressful situations and develop effective, creative solutions

Ability to meet the physical requirements of the position

Good physical health verified by a health screening, including chest X-ray or intradermal test in accordance with state regulations

Ability to read, write, and speak English; ability to explain work duties to staff

Ability to follow oral and written instructions

Strong communication skills with physicians, healthcare professionals, residents, families, and staff

Demonstrates compassion, empathy, and genuine concern for residents and families

Current First Aid and CPR certification
