Coordinate and direct the provision of home nursing care according to physicians' orders and agency policies and procedures.
Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
Conduct home visits and attend periodic office meetings.
Possess reliable transportation, a current driver's license, and agency-required liability insurance.
Manage an average of 5-8 patients per day, equating to roughly 30 points per week.
Handle various cases, including Start of Care, Wound Care, and Follow-up Visits.
Discuss weekend and on-call requirements as they vary by location.
Cover a territory with a mileage radius of up to 75 miles from the assigned home clinic, with mileage reimbursed at the IRS standard rate.
Understand that each type of visit and activity (documentation, travel, etc.) holds a different point value.
Complete all documentation within 24 hours of the appointment with the patient.
Be open to traveling up to 75 miles away from the assigned care center and participate in on-call rotations every 3-4 weeks.
Attend weekly meetings at the clinic and adhere to productivity expectations of 32 points per week.
Be proficient in OASIS and have used the HomeCare HomeBase EMR within the last 6 months or complete a retraining course if not.
