$16.00/hr - $20.00/hr Role Overview

The Case Manager supports individuals and families experiencing housing instability, with case management focused on housing stability, placement, coordination of services, crisis intervention, and community resource linkage. The role includes enrolling clients from the Coordinated Entry System (CES) into HRI programs and developing partnerships with landlords and program partners to assist clients in achieving stable housing. Responsibilities

Meet with clients at least once monthly or more frequently as required, with meetings primarily conducted in the client’s home unless otherwise approved. Conduct assessments of clients’ housing environments for safety, lease compliance, and other needs. Develop individualized housing and success plans, reviewing progress and determining actions to resolve barriers. Work outside of the office, in the community, for most scheduled work hours. Participate in case conferences, team, staff, and community meetings and committees as scheduled. Develop and foster landlord relationships, including marketing HRI programs, facilitating smooth tenant-landlord relations, and providing conflict resolution services to prevent evictions. Provide direct assistance for case management activities to achieve shelter diversion or smooth transition from emergency shelter to affordable housing. Complete all applicable eligibility and enrollment documentation, including rent calculations, rent reasonableness, fair market rent, and required signatures. Assist rent-burdened families by negotiating lease addendums to adjust costs based on household affordability. Conduct required housing unit inspections in accordance with regulations, laws, and program requirements. Respond promptly (within 2 business days) to phone calls, emails, and in-person visits, especially for emergent issues. Maintain current knowledge of all HRI programs, eligibility requirements, and services. Partner with organizations and property owners to provide collaborative referral and supportive services. Act as the agency’s Fair Housing representative when assigned. Document all client and agency-related activities accurately in all applicable systems within 2 business days. Communicate all information, decisions, changes, and essential updates to clients in a formal and professional manner. Assist with data collection for reporting and program development as needed. Review and comply with all policies, procedures, regulations, and laws related to client services and agency objectives. Adhere to all agency policies, local, state, and federal laws, and regulations. Act with compassion, empathy, and care for people experiencing homelessness and housing crisis. Maintain confidentiality and privacy of client and agency information at all times. Qualifications

High School Diploma required; Bachelor’s degree in social work or related field preferred. Minimum of 2 years of experience in case management required; lived experience with homelessness or housing crisis preferred. Strong communication, organizational, and negotiation skills. Ability to work independently and as part of a collaborative team. Knowledge of local housing resources, programs, and regulations.
