Role Description This is a full‑time, on‑site role as a Clinical Liaison located in Columbia, TN. The Clinical Liaison will serve as a vital link between Gentiva Hospice and the healthcare community, building relationships with referral sources to promote hospice services. This role involves conducting patient assessments, coordinating care plans, educating healthcare providers and families about hospice services, and collaborating with clinical staff to ensure the delivery of high‑quality care to patients.
Qualifications
Experience in Case Management and Home Care to effectively assess patient needs and coordinate care plans
Strong skills in Sales and Business Development to build and maintain referral relationships and promote hospice services
Knowledge of Rehabilitation practices to support patient care and recovery goals
Excellent interpersonal, communication, and organizational skills
Ability to work collaboratively with healthcare professionals, patients, and families
Relevant licensure or certification in nursing, healthcare, or a related field is preferred
Experience in hospice or palliative care is considered an asset
Valid driver's license and reliable transportation for local travel
