Under the supervision of the Specialized Health and Social Services (SHSS) program supervisor. Provides complex and comprehensive public health social work and case management services for children with special health care needs and their families and children and high‑risk pregnant women who receive services or are seeking services from local, state, and federal programs. Conducts Personal Care Services and Community First Choice assessments and reassessments. Provides information and referral services to families and the community. Conducts presentations and communicates program services to consumers/public in a positive and accurate manner. Provides training, consultation and technical assistance to program contractors and program providers as needed. Attends team and professional development training including staff meetings. Works as a team member. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Completes administrative duties and monthly reports. Attends work on a regular and predictable schedule in accordance with agency leave policy. Employee actively participates and/or serves in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Essential Job Functions
Case Management Functions:
Conduct home visits for comprehensive assessment of home environment, psychosocial, economic, and special medical needs of the child and family.
Provide case management services for families in CSHCN‑SP, BDES, NBS, NBHS, CLPPP, PCS/CFC, CHIP, CPS/Foster Care, CPW programs.
Assist families with eligibility determination and intake, family needs assessment, service plan, follow‑up services.
Coordinate services with Medicaid programs, local Intellectual and Developmental Disability Authority, primary care physicians and other medical providers.
Maintain case record documentation and ensure confidentiality per HIPAA and state agency guidelines.
Use software for database entry, word processing, spreadsheets, e‑mail, internet. (45%)
Personal Care Services/Community First Choice:
Conduct assessments to determine eligibility and need for PCS/CFC.
Document on Personal Care Assessment and CFC Addendum Form, submit authorizations to Texas Medicaid Health Partnership.
Coordinate services with Medicaid or local ICDHA, maintain accurate documentation, participate in fair hearing process for denials (15%).
Communicate with providers, PDN providers, Medicaid programs, discharge planners prior to hospital discharge.
Prepare documentation and participate in fair hearing process for PCS/CFC denials and modifications.
Community Organization/Resource Development:
Participate in community organization functions, collaborate with community entities on health, social work, case management issues.
Represent DSHS on Community Resource Coordination Group meetings, coalitions, interagency groups, advocacy groups.
Participate in delivery of community disaster mental health services, provide program presentations to the public (15%).
Professional & Team Development:
Attend regional and program staff meetings, trainings, conferences relevant to job for licensing requirements, provide backup and support to team members.
Continue professional development in public health Social Work/Case Management practice, participate in performance evaluations (10%).
Administrative and Other Duties as Assigned:
Complete administrative duties, monthly reports, special assignments, keep timely professional records.
Supervise Social Work Interns, support disaster response and/or recovery or COOP activation as required (10%).
Maintain acceptable driving record, possess valid Class C Texas driver’s license or equivalent, provide proof of auto insurance (10%).
Technical Assistance/Quality Assurance/Trainings:
Provide consulting, presentations, training to service providers/contractors for programs such as CSHCN‑SP, CPW, Family Support Services.
Conduct monitoring site visits to evaluate and provide technical assistance to providers, contractors, potential providers on case management issues.
Perform public relations with providers, recruit new providers, assist in training new staff (5%).
Knowledge Of

Registrations, Licensure Requirements Or Certifications
Current license to practice Social Work in the State of Texas.
Must possess own transportation and a valid Texas driver’s license or equivalent; provide proof of auto insurance.
Initial Screening Criteria
Undergraduate or Graduate Social Work degree granted by an accredited four‑year university degree program.
