Licensed Practical Nurse (LPN) – Topaz Fiscal Services LLC

We are seeking a licensed practical nurse to provide direct nursing care to residents and oversee the daily duties of nursing assistants in compliance with all applicable Federal, State, and Local regulations.

General Purpose Provide direct nursing care to residents and supervise nursing assistants, ensuring adherence to current standards, guidelines, and regulations that govern our facility.

Essential Job Functions Administrative Functions

Direct the day‑to‑day functions of nursing assistants in accordance with rules, regulations, and guidelines governing long‑term care.

Ensure all nursing personnel comply with written policies and procedures.

Plan shift services, programs, and activities with nursing staff and support personnel.

Verify compliance with the Nursing Policies and Procedures Manual.

Prepare written and oral reports about shift activities as required.

Coordinate with resident services to maintain a comprehensive care regimen.

Confirm nursing personnel comply with job descriptions.

Participate in quality assurance program development and implementation for the nursing department.

Participate in government agency inspections and surveys.

Review and update resident discharge plans as needed.

Assist in planning the nursing portion of discharge plans.

Admit, transfer, and discharge residents as required.

Complete accident/incident reports when necessary.

Charting and Documentation

Complete and file required forms/charts during admission, transfer, and/or discharge.

Encourage physicians to review, record, and sign treatment plans, progress notes, and orders.

Receive and record telephone orders on the Physicians’ Order Form.

Transcribe physician’s orders into resident charts, medication cards, and care plans.

Chart nurses’ notes informatively and descriptively.

Record new or changed diet orders and forward information to the Dietary Department.

Report discrepancies in physician orders, diet changes, or charting errors to the Nurse Supervisor.

Complete transfer forms per established procedures.

Document unauthorized discharges appropriately.

Perform routine charting duties in accordance with policies and procedures.

Sign and date all entries in the resident’s medical record.

Reflect care plan adherence in nursing notes.

Medication Administrative Functions

Prepare and administer medication as ordered by the physician.

Verify resident identity before administering medication/treatment.

Prevent medication mix‑ups between residents.

Ensure adequate floor stock of medications, supplies, and equipment.

Order prescribed medications, supplies, and equipment as needed.

Maintain accurate narcotic records for the shift.

Report drug and narcotic discrepancies to the Nurse Supervisor.

Review medication cards for completeness and accuracy.

Notify physicians of automatic stop orders before the last dosage.

Immediately notify the Nurse Supervisor and attending physician of any medication administration errors.

Nursing Care Functions

Deliver and maintain optimum resident care and comfort through current nursing practices.

Respond promptly to call lights and resident needs.

Make rounds with physicians as necessary.

Consult with resident physicians on care, treatment, and rehabilitation.

Review resident charts for treatments, medication orders, and diets.

Ensure CNAs administer treatments properly and monitor resident status.

Notify physicians of accidents or incidents.

Inform physicians and responsible parties of changes in resident condition.

Perform professional services (e.g., catheterization, tube feedings, suction, dressing changes, colostomy care, blood draws, range‑of‑motion exercises, end‑of‑life care).

Monitor seriously ill residents as needed.

Review care plans daily to verify appropriate care.

Ensure CNAs refer to resident care plans before providing care.

Other Duties

Attend in‑service education programs to meet facility educational requirements.

Familiarize yourself with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures.

Maintain confidentiality of resident and facility records.

Protect residents from neglect, mistreatment, and abuse.

Protect residents’ personal property.

Perform additional duties as directed by the supervisor or administrator.

Minimum Qualifications

Licensed Practical Nurse with a current state license.

Excellent technical, assessment, and documentation skills.

Good leadership qualities.

Organized and detail‑oriented work performance.

Strong communication skills and self‑discipline.

Genuine caring for and interest in elderly and disabled residents.

Compliance with Residents’ Rights and Facility Policies and Procedures.

Ability to perform work tasks within the physical demand requirements outlined below.

Ability to perform essential duties as described.
