Occupational Therapist (OT) – Skilled Nursing Facility
Join Flagstar Rehab as an Occupational Therapist in our state‑of‑the‑art Skilled Nursing Facilities throughout NYC and surrounding areas. Conduct evaluations, create individualized treatment plans, supervise Certified Occupational Therapy Assistants (COTAs), and deliver therapy services for conditions such as TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses. Document patient progress using innovative EMR software.
Compensation Hourly Pay – W2 $44‑48 · 1099 $55‑61
Job Duties
Provide efficient evaluations and create individualized treatment plans for COTAs to follow.
Supervise COTAs daily.
Deliver quality therapy services to individuals in an SNF setting requiring care related to TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating conditions.
Monitor and document patient progress with the use of innovative EMR software.

Current NYS OT license and registration.
EMR competence.
Meets all state and federal requirements for U.S. employment in a nursing facility.
