RN Infusion Nurse - Per Diem - Westminster Option Care Home Health LLC, the largest independent home and alternate site infusion services provider in the United States, is seeking a licensed registered nurse to coordinate and provide intermediate direct patient care to patients in the home or alternate infusion suite. The role ensures patient safety and continuity under a physician’s plan of care and works under structured supervision.

Job Responsibilities

Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organize and participate in the provision of direct patient care, perform treatments, administer medications, and educate patients and families/caregivers. Modify plan of treatment in response to changing patient status or physician orders.

Assess patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtain and clarify physician orders for plan of treatment revisions; inform physician promptly of significant changes in patient’s condition and provide written summary within supplemental order.

Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.

Teach using patient-centered care approach to help patients and families achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.

Communicate with Option Care’s clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepare clinical documentation in real-time during visits and submit to nursing department to comply with established billing timelines. Complete all documentation legibly and apply approved abbreviations and documentation error correction practices per policy.

Coordinate discharge planning and prepare discharge summaries with patient instructions and thoroughly report patient care needs, progress, and goals when transferring care.

Demonstrate compliance with agency operations, policies and procedures, professional standards, local, state, federal regulations, guidelines, and accreditation standards.

Maximize work efficiency through computers and other automation technologies to validate plan of treatment orders, communicate patient care provided, and follow assigned visit schedule.

Respond promptly and appropriately to patient requests. Initiate emergency procedures as necessary.

Accept accountability for own practice through ethical and professional conduct. Follow established programs and practices reflective of mission, values, and objectives.

Observe legal and ethical guidelines for safeguarding confidentiality of patient and proprietary information including HIPAA compliance.

Speak knowledgeably about scope of services and instruct patients about related financial obligations.

Attend required training modules, in‑services, and continuing education to maintain competency and professional licensure for demonstration of knowledge regarding care and management of patients in the home and/or alternate care settings. Provide oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in absence of Supervisor or Nurse Manager as applicable in accordance with state and federal regulations.

Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities as assigned.

Participate in nursing department on‑call responsibilities.

Perform other related duties as directed by supervisor.

Additional Responsibilities (If Applicable)

Insert PICC/Midlines per physician order.

Troubleshoot or de‑clot obstructed central line catheters.

Monitor PICC/Midline and educate on prevention of line infection and routine care.

Supervisory Responsibilities No supervisory responsibilities (hiring, scheduling, evaluations). In absence of supervisor or nurse manager, may act as temporary supervisor per state and federal regulations.

Basic Education and Experience

Active and unrestricted RN license in the state of practice.

Minimum of 1 year of nursing experience.

Current CPR certification required.

Basic Qualifications

Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs and service needs. Advanced certification and training as applicable.

Required licensure to operate a motor vehicle in the state of practice; access to a vehicle for business travel with proof of liability insurance.

Basic knowledge of computer operating systems and software applications; ability to apply nursing technology tools for communication and documentation.

Physical Demand Requirements

Ability to lift up to 50 pounds (max 75 pounds). May involve walking, standing, crouching, kneeling, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required.

Potential exposure to blood, bodily fluids, sharps, hazardous substances, extreme temperatures, unpredictable home environments.

Safety requirements include closed toe shoes, facemask, goggles, gown and/or gloves and functioning communication device.

Travel Requirements

Willing to travel up to 100% of the time for business purposes.

Preferred Qualifications & Interests

Bachelor of Science in Nursing (preferred).

1–2 years of previous infusion nursing experience (preferred).

Salary and Benefits Salary to be determined by applicant’s education, experience, knowledge, skills, and abilities. Pay range: $42.72–$71.21 per hour.
