Qualifications
Licensed Occupational Therapist
Open to applicants of all experience levels
Key Responsibilities
Screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following regulatory and clinical standards.
Utilize standardized assessments and evidence‑based practice to support clinical interventions.
Provide rehabilitative, skilled and medically necessary treatment interventions per regulatory and clinical requirements.
Administer rehabilitation procedures including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
Assess patient progress and adjust treatments accordingly in the clinical record.
Evaluate treatment outcomes for effectiveness; modify services and refer as appropriate.
Provide consultation and counseling to patients, families, caregivers, and other service providers.
Communicate with supervisor and health team about patient progress, barriers, and treatment plans.
Provide comprehensive discharge summary of services per regulatory requirements.
