Job Summary The Social Services Designee is responsible for fostering a supportive, respectful environment that enables residents to maintain individuality, independence, and dignity. This role focuses on identifying and addressing residents’ psychosocial, emotional, and mental health needs while ensuring compliance with federal, state, and local regulations and the NASW Code of Ethics.
Key Responsibilities
Complete social histories and psychosocial assessments for each resident
Accurately complete and utilize MDS sections to guide care planning
Maintain thorough, timely documentation in compliance with regulations and company policy, including:
Care plans
Progress notes
Behavior tracking
PASARR and discharge planning
Advance directives and psych service documentation
Advocate for resident rights and support self‑determination
Educate residents and families on care options and alternatives
Collaborate with the interdisciplinary care team to develop and implement individualized care plans
Monitor and document cognitive, emotional, and psychosocial changes
Qualifications
Certified, licensed, or registered in the state of practice (required)
Bachelor’s Degree in Social Work or related human services field (required)
Sociology, Psychology, Special Education, Rehabilitation Counseling, or similar
1 year of supervised social work experience in a healthcare or geriatric setting
Skilled Nursing Facility experience preferred
