Position Summary:
This is a field‑based role for a Certified Nurse Practitioner (CNP or AGNP) interested in wound care and long‑term primary care. No certification in wound care? No problem. We’ll help you get there—and cover the cost.
Key Responsibilities
Visit multiple senior care sites each week.
Deliver in‑place wound evaluations and follow‑up care.
Participate in care coordination and medication reconciliation.
Build trusted relationships with residents, families, and facility teams.
Qualifications
Current MN CNP license.
Wound care certification or willingness to obtain.
Background in long‑term care or family/geriatric practice.
Must have reliable transportation.
Military Occupational Classification (MOC) Codes

Marines: NP
Coast Guard: HS
