Vantage Care, LLC is seeking a qualified PHYSICAL THERAPIST to join our team of dedicated healthcare professionals.

The Physical Therapist will plan, organize, develop, and direct our facility’s Physical Therapy Services to ensure the highest quality resident care.

Responsibilities Administrative & Surveillance Functions

Plan, develop, organize, implement, evaluate, and direct physical therapy programs and activities in compliance with regulations.

Meet with administration, medical and nursing staff to plan therapy services.

Represent the facility at meetings and participate as directed.

Develop and maintain physical therapy standards and documentation.

Assist in standardizing methods, maintaining reference library, and developing policies and procedures.

Review physical therapy policies, procedures, manuals, and job descriptions annually.

Interpret policies to personnel, residents, family members, and respond to discharge planning needs.

Assist the Quality Assurance Coordinator and Committee in developing and implementing plans of action to correct deficiencies.

Maintain treatment records, resident files, and progress notes.

Assist with arranging transportation to other facilities when necessary.

Ensure business office receives charges for physical therapy rendered to residents.

Make written and oral reports/recommendations to the administrator regarding department operations.

Maintain liaison with families and residents and interview as necessary.

Perform administrative requirements such as completing forms and reports.

Other duties as directed by the administrator.

Committee Functions

Serve on and attend various facility committees as required.

Provide written and oral reports of physical therapy programs and activities as required.

Evaluate and implement recommendations from established committees related to physical therapy services.

Personnel Functions

Determine staffing needs of the physical therapy department.

Recommend the number and level of personnel to be employed.

Assist in recruitment, selection, and training of personnel.

Develop work assignments and schedule duty hours.

Maintain procedures for ensuring personnel have valid licenses.

Review complaints and grievances.

Report to the administrator concerning department operation.

Maintain working relationship with medical profession and other health organizations.

Develop coordination with other departments and report occupational exposures.

Therapy and Consultation Functions

Review requests for physical therapy and physicians’ orders.

Interview resident and schedule therapy as ordered.

Evaluate type of therapy after consulting with physician.

Participate in development and implementation of resident assessments and care plans.

Brief resident of procedures, prepare necessary equipment, and assist with transportation.

Demonstrate use of medical appliances and perform therapy in resident’s room.

Reassure resident before and during treatment.

Prepare resident for treatment and administer therapy per policies and procedures.

Recommend modifications based on evaluation of progress.

Ensure therapist notes are informative and descriptive.

Make rounds with attending physician as necessary.

Encourage physicians to record and sign progress notes.

Staff Development

Develop and participate in programs for in-service education, training, and orientation.

Implement effective orientation program for new employees.

Participate in departmental studies and projects as assigned.

Attend and participate in workshops, seminars, and in-service training annually.

Safety and Sanitation

Assist in developing safety standards for the physical therapy department.

Ensure personnel, residents, visitors follow established policies and procedures.

Be alert for resident safety during therapy.

Ensure personnel wear and use safety equipment when lifting or moving residents.

Assist Infection Control Coordinator in developing, implementing, and revising aseptic techniques.

Ensure adequate supplies of personal protective equipment are on hand.

Inspect physical therapy areas for compliance at least monthly.

Equipment and Supply Functions

Recommend equipment and supply needs to the administrator.

Provide necessary material and equipment for residents.

Implement procedures to use supplies efficiently and avoid waste.

Ensure MSDSs are on file for hazardous chemicals used.

Care Plan Functions

Assist in developing physical therapy plans for individual residents.

Review physical therapy requirements of each resident and assist physicians in planning care.

Involve residents and families in planning objectives and goals.

Ensure physical therapy treatments are indicated on the care plan.

Participate in development and implementation of care plans, including quarterly reviews.

Budget and Planning Functions

Prepare and plan the physical therapy budget and submit for administrator review.

Keep abreast of economic conditions and recommend adjustments to ensure quality care.

Report suspected or known incidents of fraud relative to false billing, cost reports, kickbacks.

Resident Rights

Maintain confidentiality of all resident care information and report known or suspected incidents.

Ensure resident’s rights to fair and equitable treatment, self determination, privacy, property, and civil rights are maintained at all times.

Ensure therapy personnel knock before entering resident’s room.

Ensure personnel are knowledgeable of resident responsibilities and rights including treatment refusal.

Honor resident refusal of treatment request and report such requests to Director of Nursing Services.

Inform resident of consequences of not participating in prescribed therapy and document in clinical record.

Review complaints and grievances and report actions taken to administrator.

Maintain written record of complaints and grievances.

Allow resident to participate in planning and scheduling of treatment.

Qualifications

Bachelor’s Degree in Physical Therapy from an accredited college or university.

Minimum of two (2) years experience in a therapist capacity in a hospital, nursing care facility, or other related medical facility.

Current, unencumbered license to practice as a Physical Therapist in the state.

Ability to read, write, speak, and understand English.

Knowledge of physical therapy procedures and ability to plan, organize, develop, implement, and interpret programs and policies.

Leadership and supervisory skills with willingness to work harmoniously with and supervise personnel.

Ability to make independent decisions in a timely fashion.

Ability to plan, organize, develop, implement, and interpret goals, policies, procedures, and programs.

Proficiency in maintaining care and use of supplies, equipment, and appearance of therapy areas.

Patience, tact, cheerful disposition, and enthusiasm for working with residents.

Willingness to seek new methods and incorporate them into service delivery.

Ability to relate information concerning resident condition and maintain health and safety of workplace.

Physical and Sensory Requirements

Move intermittently throughout the workday and be able to lift a minimum of 25 pounds.

Speak and write English in an understandable manner.

Cope with mental, emotional, and physical stress of position.

See and hear adequately for functional performance.

Work independently with flexibility and personal integrity.

Relate to and work with ill, disabled, elderly, and emotionally upset individuals.

Assist in evacuation of residents during emergencies.

May be required to perform tasks involving exposure to blood or body fluids.
