Join Our Team as a Hospice and Palliative Care Nurse Practitioner. Are you committed to providing quality, compassionate care? We seek nurse practitioners dedicated to creating meaningful patient experiences.
As a hospice and palliative nurse practitioner, you will care for seriously ill patients to alleviate burdensome symptoms and align medical treatments with patients' goals and values. You will collaborate with a team of clinicians and teammates to deliver the best care possible wherever patients call home. As an active member of the medical community in your territory, you will provide ongoing palliative care, hospice service, and education.

Qualifications
A heart to serve patients and families and a passion for providing the best possible care.
Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred.
Experience: 2 years within the last 5 years in chronic disease management/long-term care; hospice background preferred.
Required: Reliable transportation; ability to sit, stand, bend, move intermittently and lift at least 80–100 lbs and bear the weight of an average adult effectively.
