As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge. How You Will Make a Difference
Assist residents and their families with social, emotional, and family issues. Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies. Participate in resident care planning. Participate in the development of Discharge planning and prepare resident for the transition. Serve as a general source of information for residents and their families. What You Need To Make a Difference
Bachelor’s or Masters Degree in Social Work or Psychology. 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting. High level of professionalism and ability to maintain confidentiality. Benefits
