Job Summary The Habilitation Coordinator plans, coordinates, and monitors active treatment services that promote independence for individuals served through a person-centered approach. This role ensures consistent, high-quality implementation of individual programming and behavior support plans while providing clinical oversight and leadership within Echoing Connections. As part of the supervisory management team, the Habilitation Coordinator – Degreed supports program direction, provider collaboration, and regulatory compliance to ensure effective and meaningful services.

Responsibilities Coordinate, implement, and monitor active treatment programming, including individual service plans and behavior support plans. Develop, evaluate, and train staff on habilitation programs and interventions to ensure quality and consistency. Ensure all habilitation documentation is accurate, complete, and compliant with regulatory requirements. Foster a person-centered environment that empowers individuals to build skills, relationships, and independence. Develop and oversee structured activity schedules aligned with individual goals and formal programming. Serve as a liaison to providers and represent Day Habilitation services at ISP and related meetings. Supervise and guide staff in the implementation of active treatment and behavior management strategies. Operate agency vehicles and transport individuals as needed to ensure program coverage. Assist leadership with compliance, audits, MUIs, incident reporting, safety procedures, and emergency preparedness. Participate in ministry initiatives, special projects, committees, and fundraising activities as assigned.

Qualifications Bachelor’s degree in social services, rehabilitation, education, or a related field required. Minimum of one year of experience in habilitation services, IDD, or a related field required. Prior supervisory or lead experience preferred. CPR, First Aid, and Medication Administration certifications required. Ability to support individuals with IDD through a professional, person-centered, and Christian-based approach, including direct care. Strong communication, problem-solving, and conflict management skills. Valid driver’s license with an insurable driving record and reliable transportation. Demonstrated Christian background and ability to support the mission and values of the ministry.
