Position Summary The Social Worker will conduct and prepare psychosocial assessments, offer therapeutic interventions, and contribute to the discharge planning process for patients and families receiving services through the Lindner Center of HOPE. In addition to the daily tasks, inpatient social workers will assist patients and/or their families/responsible parties in coping with issues related to hospitalization, illness, diagnosis, and/or life situations while ensuring that post-hospitalization needs are met.
Major Duties and Responsibilities
Will provide social work services on the assigned unit but may also include providing social work services on the other inpatient unit as needed.
Conducts and prepares psychosocial assessments for patients receiving services at LCOH. During the assessment process, Social Work will identify and acknowledge emotional, social, and environmental strengths and challenges while addressing the needs of patients, families, and responsible parties relating to diagnosis, illness, treatment, and life situations.
Will participate in the LCOH clinical team which contributes to the formulation, development, and implementation of the patient’s treatment plan utilizing appropriate social work interventions. Interventions may include crisis intervention, brief therapy, cognitive‑behavioral, and dialectical‑behavioral methods.
Will document in the Electronic Medical Record and paper chart. Documentation is timely, legible, concise, and pertinent and meets the professions and department’s documentation standards. Oral reporting is clear, concise, relevant, and timely. Perform all required data collection for the department accurately and within the expected time.
Will address discharge planning needs across a multidisciplinary care continuum which includes interacting with consulting physicians and other health care providers.
Will provide education to patients and families around issues related to the adaptation of the patient’s illness and/or life situations.
Will participate in clinical supervision with the Manager of Social Work on a scheduled basis.
Will maintain a working knowledge of and collaborate with community agencies; and participate in area activities related to health and social services while providing brief overviews to colleagues on clinical education attended.
Will maintain licensure and pursue current information related to mental health and social work. Participate in continuing education opportunities to further enhance job‑related knowledge.
Will participate in department and LCOH quality improvement efforts and meet peer review standards.
Will meet competency skills for evaluations in social work practice.
Will accept students for field placement and participate in their learning experience.
Will participate in scheduled staff meetings.
Position Qualifications
Master of Social Work degree from an accredited school of social work.
Licensed Social Worker or Licensed Independent Social Worker by the State of Ohio Social Work Board.
1-3 years experience in a health care setting; Psychiatric/mental health experience required.
Solid knowledge of social work practice; strong assessment, interview, and counseling skills.
Strong people skills: ability to collaborate well.
Ability to enthusiastically follow and model the Lindner Center of HOPE mission, vision, and values.
Physical Requirements The physical demands of this position are consistent with those performed in a normal office environment, including occasional lifting, carrying, pushing, or pulling, sufficient manual dexterity to operate standard office equipment, and the ability to use a computer. Must have an ease of mobility to be able to frequently move through the Lindner Center of HOPE.
All candidates extended conditional offer of employment will be subject to will conduct a WebCheck (BCI & FBI Fingerprinting). Fingerprints will be submitted to the Bureau of Criminal Investigation (BCI) and the Federal Bureau of Investigation (FBI). The reports from these agencies will include criminal record information. Lindner Center of HOPE will follow the requirements for employment based on the State of Ohio Administrative Code 5122-30-31, and any other regulatory requirements regarding criminal background checks. Lindner Center of HOPE also reserves the right to obtain Consumer Reports and/or Investigative Consumer Reports as defined in the Federal Fair Credit Reporting ACT (FCRA).
