Brief Job Description This position is available for morning, evening, and night shifts across multiple homes. It may qualify for additional pay for evenings, nights, weekends, and maximum‑security locations.

Join a team that values and respects the residents. The State Supported Living Centers serve individuals with intellectual and developmental disabilities who are medically fragile or have behavioral issues, providing campus‑based direct services at 13 centers statewide.

Free CEUs and updated resources through Lippincott access are provided for all nurses.

The Direct Care RN performs routine level nursing care, providing assessment, care and treatment for residents at a State Supported Living Center (SSLC). Duties include evaluating, treating, and documenting healthcare needs, reporting to the appropriate personnel, communicating with direct care staff, physicians/PCPs, administration, and other nursing staff to ensure optimal interdisciplinary care. The RN notifies the supervisor and PCP of trends and concerns, completing data entry, required documentation, and record maintenance.

Assists LVNs and Certified Medication Aides (CMAs) as needed, recommends improvements, participates in quality assurance/quality improvement initiatives, and provides treatment to HHS employees injured in the course of employment. Responds to emergency situations to ensure safety and provides care to residents and staff. The RN works under moderate supervision of the Nurse Manager or Nursing Operations Officer with limited latitude for independent judgment.

Essential Job Functions (EJFs)

Attends work on a regular basis, may be required to work a specific shift schedule or rotating schedule, extended shift, and/or overtime in accordance with agency leave policy, performing other duties as assigned.

May serve in an on‑call status on a rotating schedule as required to meet operational needs of the facility.

Performs routine nursing tasks and procedures per HHSC, SSLC, and state regulations, including tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments, specimen collection, and laboratory screening.

Administers medications, immunizations and treatments, observes patients for reactions, makes rounds or home visits as needed, and works in other locations within the SSLC.

Monitors individuals’ weight, diet, feedings, and vital signs; monitors for signs and symptoms of illness, using nursing judgment to provide adequate care and initiates acute care plans.

Assists in evaluating nursing intervention outcomes and provides input to the healthcare team with recommendations for improvements.

Instructs direct support professionals on monitoring individuals after procedures using instruction sheets, such as post‑sedation monitoring.

Documents and reports treatments and observations accurately in the individual’s medical record (electronic or paper chart) following SSLC procedures and protocols; prepares required reports.

Assists LVNs/CMAs with implementing protocols, procedures, and programs to improve overall health and safety of staff and individuals, including completing follow‑up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and medication variances.

Provides support to the Nurse Manager’s, reviewing and following-up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temperature logs, and glucose controls.

Assists the Infection Control nurse with measures preventing disease spread and assists the Quality Assurance Nurse in implementing therapeutic nursing programs; participates in facility QA/QI initiatives.

Responds to emergency situations ensuring safety of individuals, follows safety and infection control procedures, provides educational and safety training to SSLC staff upon request, and treats HHS employees injured in the course of employment.

Other duties as assigned, including participation in disaster response and/or recovery or Continuity of Operations (COOP) activation; may require alternate shift pattern or location.

Knowledge, Skills and Abilities (KSAs)

Knowledge of nursing techniques and procedures; health care laws and regulations; medical diagnoses and procedures; accepted medical treatment programs; community health and nursing care principles, practices, and procedures.

Knowledge of state and federal laws, Intermediate Care Facility for individuals with intellectual disabilities (ICF‑IID) rules, regulations, policies, and procedures related to State Supported Living Center or ability to gain that knowledge rapidly.

Knowledge of nursing care standards, the Settlement Agreement, and Healthcare Guidelines, or ability to gain knowledge rapidly.

Skill in the care and treatment of patients.

Skill in using computer/software and electronic health record systems.

Excellent verbal and written communication skills; ability to present ideas clearly and concisely in both written and oral communications.

Ability to provide patient care; prepare and maintain records; interpret policies, procedures, regulations; recognize patterns of medical necessity treatment.

Ability to demonstrate a working knowledge of emergency procedures and equipment.

Ability to interact appropriately with individuals with intellectual/developmental disabilities, their families, and staff members.

Ability to work independently and productively with all levels of staff.

Ability to provide support and coaching to LVNs, CMAs, and DSPs.

Ability to make decisions and recommendations to SSLC leadership including on‑call administrators.

Registrations, Licensure Requirements or Certifications Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).

Initial Screening Criteria

Work experience as a practicing registered nurse is required.

At least six (6) months of experience as a practicing registered nurse in a health agency, hospital, or health‑care facility is preferred.

Bachelor’s Degree is preferred.
