Licensed Practical Nurse – Peekskill (NY) – Full‑Time – Sun River Health Employer:

Job Type:

Full‑Time.

Pay:

$29.00 – $33.00 per hour.

Sign‑On Bonus:

$2,700 for eligible external applicants (Terms and Conditions Apply).

Summary The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, Regional Nurse Manager, or Medical Provider. The LPN performs clinical interventions, collects clinical data, documents nursing observations and interventions, administers prescribed medication, and follows Sun River Health standard procedures and workflows.

Responsibilities

Accurately collect clinical data on patients and demonstrate competency in performing clinical interventions; report abnormalities to the Medical Provider or RN.

Document nursing observations, interventions, and patient responses to treatment in patient records.

Perform venipuncture for collection of blood specimens and facilitate prompt and accurate processing and distribution of specimens.

Ensure timely and appropriate patient care within the delivery system; facilitate internal and external referrals in collaboration with the RN.

Attend internal and external educational seminars and staff trainings; perform all related duties as assigned or unrelated duties during emergencies.

Administer prescribed medication to clients and keep accurate records in accordance with law and Sun River Health policies and procedures.

Implement and follow Sun River Health standard procedures and workflows, including medication management, pre‑visit planning, team huddles, accurate and timely completion of Nursing Quality Logs, exam room readiness, ordering and storing supplies and equipment.

Provide age‑appropriate and culturally sensitive care; use medical interpreters as indicated.

Preceptor to students, new hire candidates, and other clinical team members/ staff when needed.

Utilize standing orders and attend mandated training; demonstrate competence on all nursing procedures, including relevant lab procedures.

Perform direct patient care duties of an LPN: interview patients for health history, prepare equipment, assist practitioner during examination and treatment, document in medical record, gather information from patients as needed.

Educate patients for health promotion and disease prevention to improve health outcomes.

Qualifications & Minimum Licensure

Required:

High school diploma, GED, or equivalent; nursing required.

Minimum work‑related experience: 0–1 year.

Preferred work‑related experience: 2–4 years.

Valid New York State LPN license.

CPR/BLS‑HCP certification.
