Pay:

$63,000 - $79,000 annually, based on experience! Relocation Bonus Offered!

Schedule:

Monday – Friday 8 a.m. – 5 p.m. Travel in the community to and from participants’ homes may be required.

Responsibilities

Screen and assess participants’ nutritional status; use data to plan and implement appropriate nutrition interventions.

Communicate interventions to the Interdisciplinary Team to ensure nutritional needs are met.

Provide nutritional education for participants and caregivers.

Coordinate serving of meals and monitor kitchen facilities to maintain standards.

Manage contracted meal services to ensure meals meet participants’ needs.

Educate other health‑care professionals when appropriate.

Provide direction and supervision to Dietary Aide.

Deliver quality Nutrition Care through the Academy of Nutrition and Dietetics evidence‑based process for each participant.

Conduct in‑person reassessments yearly, every six months for active Plan‑of‑Care developers, and when status changes.

Attend and participate in Interdisciplinary Team meetings; collaborate on care planning and 24‑hour care coordination.

Monitor monthly weights, medical diagnoses, drug‑nutrient interactions, chewing/swallowing changes, mood, hydration, intake, skin breakdown; report changes.

Document nutrition notes and all activities in the participants’ medical records following Senior CommUnity Care standards.

Provide nutritional counseling and education to participants and caregivers using appropriate materials.

Develop and implement therapeutic dining programs and nutritional components of the plan of care.

Monitor meals, including intake, diet compliance, and tray accuracy.

Oversee dining program operations, inventory, and emergency dietary supply management.

Plan special events, ordering, setup, breakdown, and cleaning as needed.

Supervise and evaluate Dietary Aide’s competencies and performance.

Ensure clear job responsibilities, authority, and accountability of direct reports.

Oversee kitchen cleanliness, portion control, therapeutic diet compliance, and food purchasing/inventory.

Manage therapeutic diets and physician orders; liaise with contracted meal service to meet needs.

Assist in developing policies and procedures for nutrition and dining services.

Protect privacy and maintain confidentiality of employee, participant, and family information.

Participate in quality improvement initiatives and other committees as requested.

Qualifications

Advanced degree from an accredited college in food and nutrition or dietetics (Masters preferred).

Licensed as a Dietitian in the state; Registered Dietitian Certification required.

Minimum of one year of experience working with frail or elderly populations.

Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.

Valid driver’s license and reliable transportation.

Clear background check and drug screen.

Skills and Knowledge

Understanding of physical, mental, and social needs of frail older adults.

Strong assessment skills for nutritional needs of frail older adults.

Excellent written and oral communication.

Ability to work independently and as part of an Interdisciplinary Team.

Strong organizational skills.

Proficiency with computers and electronic devices for timekeeping, documentation, and training.
