Job Description The position of Bilingual Case Manager requires a positive, motivated, compassionate, organized, and detail-oriented individual who can work well with a population with pervasive mental health issues. This fast-paced, detailed role ensures clients are eligible for services with our agency.
Responsibilities
Facilitate a high-volume mental health intake process by managing referrals, enforcing no‑show policies, and scheduling intake appointments to ensure efficient onboarding of new clients.
Verify Medicaid eligibility to ensure potential clients have proper medical coverage and meet NYPCC criteria for admission.
Conduct an average of 40 to 45 daily interactions with new clients, demonstrating excellent interpersonal and communication skills to establish rapport and guide individuals through the intake process.
Play a pivotal role in client‑and‑therapist matching following intake completion, including reassignment of caseloads during therapist transitions, leaves, or departures, ensuring seamless continuity of care within a dynamic and fast-paced organizational environment.
Perform other duties as assigned by Supervisor.
Qualifications
Bilingual; fluent in English and Spanish
Associate degree in a related field
1 year of experience working in a social service or mental health setting (preferred)
1 year of experience using an EHR system
Experience working in a very fast-paced environment
Ability to follow workflows and multi-task during the day
Detail oriented
Exceptional customer service and communication skills
Excellent organizational and time management skills
Problem‑solving skills
Ability to work independently and as a team
Proficient in Microsoft Office, including Outlook, Word, Excel, and PowerPoint
Maintains a positive attitude and is friendly, upbeat, and has a great “can do” personality
Passionate about NYPCC’s mission and values
Additional Information Salary: $21.00 - $23.00 per hour
Compensation commensurate with experience and qualifications.
