Deliver high-quality nursing care in home health settings using the nursing process. Assess patient needs and collaborate on individualized care plans. Educate patients and families using population-relevant materials. Evaluate care outcomes and adjust plans as needed. Coordinate with interdisciplinary teams to ensure continuity of care. Participate in performance improvement and cost-effective care initiatives. Stay current with research and apply evidence-based practices. Qualifications
