Base Pay Range $230,000 - $315,000 per year
Summary A recruitment/relocation incentive may be offered to highly qualified candidate(s). The incumbent will serve as a Complex Patient Aligned Care Team (PACT) Physician located at the Daytona Beach VA Clinic in Daytona Beach, FL, assigned to the Ambulatory Care Service. Assignments to other facilities within OVAHCS and travel to the site may be required to meet the needs of the service. The position is eligible for the Education Debt Reduction Program (EDRP).
Responsibilities
Seeking an outstanding full‑time Family Practice or Internal Medicine trained physician with recent experience in VA outpatient PACT panel management to join the Primary Care Clinic team as a Complex PACT (COM‑PACT) Primary Care Provider.
Provide comprehensive gender‑specific care under the PACT model and intensive case management to a panel of patients with complex chronic medical conditions managed under PCMM guidelines to reduce recurrent ED visits and hospital readmissions.
Attend to patient education, preventive care and work well in an interdisciplinary team to provide high‑quality primary care.
Manage chronic conditions, evaluate and treat intercurrent medical conditions.
Determine medical assessment and develop treatment plans according to accepted guidelines.
Collaborate with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, continuous care to an assigned panel of patients.
Work with a multi‑disciplinary team to prevent recurrent emergency department visits and frequent readmissions.
Carry a team cell phone 7am‑9pm Monday‑Friday.
Perform disease prevention activities including gender and age‑specific screenings, mental health screenings, and health promotion activities.
Screen, evaluate and treat uncomplicated common mental health disorders and common addictions with referrals to mental health providers as appropriate.
Provide clinical oversight and participate in peer review of other primary‑care providers through periodic chart reviews.
Act as surrogate for absent primary‑care providers when needed and review CPRS view alerts.
Attend primary‑care staff meetings, departmental meetings, and medical staff meetings regularly.
Document timely (notes and encounter forms) per facility policy.
Complete all administrative work in a timely manner.
Address clinical reminders as medically appropriate.
Maintain quality of care meeting professional standards.
Utilize all Virtual Care modalities.
Provide timely follow‑up on results, including completion of view alerts.
Prescribe medication according to approved formulary and federal/state law.
Consult other specialties as appropriate according to service agreements.
Demonstrate flexibility in responding to clinic demands and maintain ethical standards.
Work schedule: Monday through Friday 8am‑4:30pm (shifts may vary).
Provide transportation/travel as required.
