Registered Nurse (RN) – Home Health – FT
Join VitalCaring Group as a full‑time Registered Nurse in our home‑health services. In this role, you’ll provide skilled care in patients' homes, help them regain independence, and experience a supportive, flexible schedule.

Responsibilities
Evaluate and assess patients to determine appropriateness for care.
Develop patient‑specific care plans using available tools, ensuring appropriate level and frequency of care.
Communicate with the care team, physicians, payors, patients, and families to address care needs and fulfill the care plan.
Deliver high‑quality skilled care and supervise other care team members.
Thoroughly document care delivery daily in our EMR system.
Contribute to a culture of accountability and teamwork.
Qualifications
Current RN license, valid driver’s license, and reliable transportation.
Preferred: One year of experience as an RN in an acute or clinical care setting, with home‑health experience preferred.
Skills for Success

Love learning and motivating patients to reach goals.
Enthusiastic about working to the highest level of the RN license.
Familiar with technology; HCHB experience is a plus.
