Job Overview
Advanced clinical social work practice in a medical setting focusing on kidney transplant patients and families. Engage in multi‑disciplinary treatment planning and coordinate complex discharge plans using Ochsner and community resources.
Responsibilities
Interview and assess patients and assist throughout patient care plan.
Identify and provide helpful resources to patients and families.
Maintain patient records and document critical information for the medical team.
Participate in on‑call rotation.
Adapt communication and behavior to the specific patient population, respecting privacy and ensuring clear explanation of services.
Remain knowledgeable on current federal, state, and local laws and compliance requirements; report unethical or questionable behavior.
Qualifications
Licensed Clinical Social Worker (LCSW) as verified by LABSWE.
Master’s degree in Social Work (MSW).
Five years related experience in a medical social work field.
Knowledge, Skills & Abilities (KSAs)
Computer skills for data entry and patient information retrieval.
Proficiency with Windows applications and keyboard use.
Effective verbal and written communication; ability to present information clearly to various audiences.
Knowledge of common disease entities, medical terminology and psychosocial effects of illness.
Excellent psychotherapeutic and interpersonal skills.
Advanced knowledge of acute and chronic medical settings, managed care and public programs (Medicare, Medicaid, etc.).
Physical and Environmental Demands
Medium work: Ability to lift/push objects up to 50 lbs occasionally.
Must sit and stand for prolonged periods; travel between facilities as needed.
Flexible schedule required (24/7, weekend, holiday, on‑call).
No significant exposure to blood or body fluids, but potential exposure to communicable diseases.
