Job Summary Provides counseling, support, and assistance in helping patients and families transition to post‑hospital care. Coordinates discharge plans, refers to appropriate community resources, and follows up after patients leave the hospital.
Responsibilities
Collaborates with multidisciplinary team to develop patient discharge plans for post‑hospice or post‑inpatient care.
Assists patients/family/caregivers in determining appropriate level of hospice care and refers to outpatient resources as needed.
Initiates plan of care and performs patient teaching. Reassesses plan of care according to physical and emotional needs of patient.
Develops plan of care based on psychosocial assessment, clinical knowledge of human development stages, terminal illness impact, mental status, and family dynamics.
Performs other job‑related duties as assigned.
Qualifications
Master’s Degree in Social Work (required in CA and HI).
One year experience in an acute or long‑term medical setting (required).
Two years experience in an acute or long‑term medical setting (preferred).
