Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County.

Sun River Health is currently seeking a part‑time Nurse Practitioner to join our teams in Coram and Patchogue.

A Nurse Practitioner, under the direction of a physician, is responsible for planning, evaluating and delivering the health care of patients on assigned service. Emphases preventative health measures and education of patients. She/he will perform other duties as assigned and in compliance with systems and job‑specific standards.

Essential Duties and Responsibilities for a Nurse Practitioner:

Takes histories, performs physical examinations, gives immunizations (as necessary) and does patient education and counseling.

Orders appropriate lab work, draws blood for lab work if needed, and performs some lab tests such as wet prep microscopic exams.

Prescribes treatment for minor illnesses, and consults with physicians regarding medication, chronic illness and referrals when appropriate.

Maintains legible and appropriate documentation and reviews records periodically. Does follow‑up of abnormal pap smears and reportable STD’s.

Attends case conferences involving patients.

Attends continuing education programs to increase skills and knowledge.

Attends provider meetings and participates in quality improvement programs and team assignments.

Identifies and categorizes each patient’s age‑specific grouping of needs, such as those infants, adolescent or geriatric patients.

Follows OSHA standards of infection control under CDC guidelines endorsed by department, organization and overseen by Infection Control Officer.

Provides safe and successful performance, including meeting productivity, quality and patient satisfaction standards.

Qualifications

Master’s Degree in Nursing.

5–8 years of experience; 4 years of business office experience, preferably in a large medical practice with increasing levels of responsibility.

8+ years preferred.

Valid license as a Nurse Practitioner in New York State and CPR/BLS certification.

Required Skills & Knowledge

Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.

Competence in delivery of health care to patients (including preventative health education).

Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.

Bilingual in both English and Spanish (orally and written) is preferred.

Job type: part‑time

Hourly salary: $74.46–$78.07 per hour
