The Social Services Designee is under the supervision of a social worker. The primary purpose of this position is to plan, develop, organize, implement, and direct activities in accordance with current Federal, State, and local standards, as well as our policies, to ensure the emotional and social needs of residents are met on an individual basis.
Responsibilities
Plan, develop, organize, and implement the overall Social Services function in the facility.
Meet with the Social Worker or Director of Nursing and other facility management personnel to plan social services during IDT meetings.
Represent the facility as a Grievance officer and collaborate with the Social Worker, Director of Nursing, and the Interdisciplinary Team to resolve issues.
Oversee behavior management and coordinate with the Interdisciplinary Team and the psychiatrist’s psychotropic dose‑reduction schedule.
Ensure the facility provides necessary behavioral, mental, and emotional health care and services to each resident.
Refer residents to ancillary services such as dental, optometry, audiology, podiatry, psychology, and psychiatry.
Maintain good working rapport with other departments and external community health, welfare, and social agencies to keep social service programs effective and meeting residents’ needs.
Monitor current Federal and State regulations and recommend necessary changes.
Review departmental policies and procedures and recommend adjustments per recent regulatory guidance.
Develop and manage a departmental budget and monitor expenditures.
Serve on facility committees as appointed by the Administrator, Social Worker, or Director of Nursing.
Collaborate with other disciplines to provide in‑service training that staff can use for behavior and related areas.
Participate in discharge planning and coordinate with external resources to ensure a safe transition for residents.
Use IDT to discuss and meet social service needs, maintaining communication with resident family members.
Schedule Interdisciplinary Team meetings for discharge planning, psycho‑social/behavioral care plans, POLST forms, and resident assessments upon new admissions, quarterly, annually, or with significant changes.
Arrange and collaborate on appointment and transportation needs for residents; arrange discharge transportation.
Provide residents and families with resources and financial aid information from government and private agencies.
Assist and direct staff when handling problematic or behavioral situations.
Store and retain grievance records for three years.
Document events promptly and accurately.
Complete assessments and observations for new admissions, quarterly, annually, and with significant changes in a timely manner.
Maintain the Theft & Loss program and store related records per state and federal regulations.
Perform all other duties assigned by the Administrator or Director of Nursing, such as delivering mail, assisting residents with voting forms, and supporting residents in bringing mail to the post office.
Education & Experience
Social Services Certification (ACSW) from an accredited program within three months of hiring.
Previous SNF experience preferred.
Preferred State Requirement: Full Covid‑19 Vaccination Up To Date.
